This is done through Dr. Blagrove's 9 week caregiver-tailored framework of personalized medication therapy navigation. This program reorients your relationship with your loved one from an overwhelming obligation to a rewarding routine. Dr. Blagrove co-creates with you the ability to care for our aging loved ones in a way that honors your needs and their values.

We will meet for one hour each week for the first four weeks of the program to establish a medication plan. This will include the development of goals, identification of barriers, medication reconciliation, medication counseling, and a creating a patient advocacy framework.

For the remaining weeks we will check in for another for 30 minutes a week. These calls will be used to coordinate, monitor, connect with resources, and provide additional support as needed (even if it's just an unbiased listening ear).
